LCD Update from Dec 2021 Changes

The LCDs released in December 2021 related to Epidural Steroid Injections have again been updated. Though all MACs introduced an LCD to address this topic, the messaging is still changing. Throughout January, eight revisions from five different MACs were made to the LCD language. Some of the changes made are backdated to the December effective dates of the LCDs.  Noridian is the only carrier we know of that has not yet adopted the new national policy.

The specifics of the updates are geared to three items:

  1. Utilization clarification of only 4 epidural sessions in a rolling 12 month period regardless of anatomical area or levels involved.
  2. Removal of M48.061 as a covered diagnosis as it was included in error. Radiculopathy must be present for coverage.
  3. Steroid dosing should be at the lowest effective amount. It is recommended not to exceed 80 mg triamcinolone, 12 mg betamethasone or 15 mg dexamethasone per session.
Clients should keep on alert watching these LCD policies and the billing articles that apply. Several of the questions that have been asked have been put in a Clarification Document A58899
